| Description: | Chronic Care Nurse (RN) is responsible for the overall tracking, management, and timely care provisions for patients with chronic medical conditions. This position works closely with onsite physicians and mid-level providers to ensure chronically ill patients are seen on a routine and scheduled basis as dictated by their disease process. Also directs and guides patient teaching activities and ancillary personnel, while maintaining standards of professional nursing. Interrelates and works effectively with patients, medical and other nursing staff, security staff at all levels, and administrative and support staff. |
